胃部溃疡 (Ulcer) 导致的疼痛是因胃酸接触到未被保护的组织所致。一般来说,进食后由于食物中和了胃酸,可以暂时缓解疼痛。但是一旦食物被消化,疼痛会再次出现。

胃溃疡的常规治疗方法经历了一次缓慢的变革。几十年前,医生开给溃疡病人的处方只是清淡饮食,即少吃辛辣食物,多吃乳制品。当时认为乳制品可以保护胃组织。但是后来发现溃疡与进食辛辣的食物无关,牛奶反而可能导致溃疡的发生,这就使得在那时可用的治疗方法只剩下手术了。后来含有镁和铝成分的抗酸药 (如Maalox) 被研制出来了。尽管如此,这些药的疗效还是不够强,不足以让溃疡完全愈合。随着胃泰美 (西咪替丁)、善得胃 (雷尼替丁)、法莫替丁等的出现,胃溃疡的治疗迈上了一个新台阶。这些 H2 受体抑制剂可以显著地减少胃酸的产生。再后来,一类更强的抑酸药 (质子泵抑制剂) 问世了,其代表药物是奥美拉唑。
当胃酸被抑制的时候,溃疡痛会快速消失,溃疡也会愈合。因此在一段时间里,这些药物被认为是治疗溃疡的 “克星”。但逐渐地人们发现停药后溃疡会屡次复发,这使得原先对这类药物的热情开始消退。如今我们认为溃疡是由幽门螺杆菌引起的。这种早先被忽略的细菌可以使胃部感染并以此减弱胃粘膜的抗酸作用,因此只有合用杀灭幽门螺杆菌的抗生素及抑酸药才能治愈溃疡。不过,杀灭幽门螺杆菌并不容易,服用抗生素并不总是有效,而且还可能会导致副作用。
胃溃疡的自然疗法包括:
- 益生菌 (Probiotics):它们是指对人体有益的细菌。最常见的益生菌是在酸奶酪中发现的乳酸杆菌。它是乳杆菌属中的一种。证据显示乳杆菌属中的多种益生菌都能够抑制幽门螺杆菌的生长。多项初步研究均发现,益生菌的这种抑制作用虽然还不够强大到可以清除幽门螺杆菌,但是它们能增强标准的抗菌素治疗效果及减少副反应。
- 蔓越莓 (Cranberry):蔓越莓被认为可以通过阻断细菌对膀胱的粘附来预防膀胱的感染。初步的证据显示其同样可以阻断幽门螺杆菌对胃壁的粘附。在中国进行的一项为期90天的双盲研究评估了每日饮用蔓越莓汁对于治疗长期幽门螺杆菌感染的疗效。结果表明食用蔓越莓可以显著的降低幽门螺杆菌在胃中的数量,其原因可能是蔓越莓汁 “冲走” 了一些幽门螺杆菌。
